Windsor Place At-Home Care Selects Vivify Health for Connected Health and Patient Engagement
Partnership includes new interface with Kansas Health Information Network to provide physicians with access to biometric monitoring data
PLANO, TEXAS — October 30, 2018 — Vivify Health, the developer of innovative virtual solutions for holistic patient care and engagement, announced today that it has partnered with Windsor Place At-Home Care, based in Coffeyville, Kansas, to deliver remote patient monitoring services to Windsor’s home care patients across the state. Windsor, which has employed remote care management since 2008, switched to Vivify from a large telehealth vendor with international operations because of the superior personalization and efficiency of Vivify Health’s solution.

Vivify Health’s efficient back-end software enables Windsor’s nurse care managers and coordinator teams to support 250 patients each while continuing to deliver good outcomes. Windsor staff received four days of training last summer and already feel comfortable using the Vivify solution.

Through this window, physicians and other caregivers can gain visibility into the condition of their patients between office visits.
